Patient Incentives Program

THE ULTIMATE PATIENT INCENTIVE PROGRAM!!!h

You will be competing against yourself for prizes. Collect points and redeem them for great prizes! Earn one point every time you:

  1. Keep your original scheduled appointment (no broken, cancelled, or rescheduled appointments).
  2. Have clean teeth and healthy gums when you arrive for your appointment.
  3. Wear appliances as prescribed (headgear, rubber bands, retainers, etc)
  4. Nothing loose or broken

If you follow all four rules, you will receive a total of four (4) points from Dr. Melanie or staff. You can also earn points for:

  1. Every “A” on your report card = 2 points
  2. Every “B” = 1 point
  3. A signed certificate from your dentist at your routine cleaning = 5 points
  4. Refer a friend = 10 points

If for some reason you do NOT meet all of the above requirements, you may still receive points in the other categories. For example, if you forget to bring your headgear to your appointment, you can still receive three (3) points, just not all of the possible four. You will not be punished for violating one single rule but you will be rewarded for doing your best; the better you do, the more you win!!!
